Wide channels on 2860AC 5Ghz was created by psketch
When I select Mixed mode (11a, 11n, 11ac) on my 2860AC, and then look at a channel analyser (Inssider etc.), the channels only show up as 20Mhz, even though I've set to use 20or 40Ghz channels. If I change to 11a or 11n only, then I see 40Ghz channels available. Any ideas why?

Which software are you using? I expect Inssider would be able to see 40MHz channels but I know that some older versions of Inssider can't see 80MHz channels.
You could try Acrylic wifi's scanner software to see if that sees the channels in the same way.
